New Student Registration Information
In order to register a child for school, parents MUST bring the following documents:
· Picture ID of parent or guardian who is registering the student (driver’s license)
· Proof of residency (i.e. lease agreement, property deed, sales contract, property tax statement for homestead.)
· Birth certificate – the original county or state issued certificate, NO copies.
· Social Security number (preferred, but optional) If you do not furnish your child’s Social Security number, the child will be assigned a state ID number.
Immunization record including:
(Note that these are immunization requirements for the 2012-13 school year.)
All Grades: Four doses of polio vaccine (unless 3rd dose was received on or after 4th birthday)*
All Grades: Five doses DTP/DTaP vaccine (unless 4th dose was received on or after 4th birthday)*
Grades 3-5: One dose MMR vaccine received on or after 1st birthday and
One dose of measles vaccine (dose may be part of a second MMR)
Kindergarten, 1st and 2nd Grades: Two doses of MMR, both on or after the first birthday.)
All Grades: Three doses hepatitis B vaccine
Grades 3-5: One dose of varicella vaccine received on or after 1st birthday
Kindergarten, 1st and 2nd Grades: Two doses of varicella vaccine, both on or after the first birthday.)
Kindergarten, 1st and 2nd Grades: Two doses of hepatitis A vaccine, both received on or after the first birthday.
*DTP/DTaP/DT/Td and Polio series must include a dose on or after the fourth birthday to be complete.*
Receipt of a dose up to (and including) 4 days before the birthday will satisfy the school entry immunization requirements.
Serologic confirmation of immunity or documentation of previous illness may substitute for vaccination.
In order for a student to attend school, his/her immunization record must be up to date, unless the parent has filed an exemption with the Texas Department of Health in accordance with the Texas Education code (Chapter 38§38.001).
STUDENTS CAN BE REGISTERED WHEN ALL DOCUMENTS ARE COMPLETE.
In order to register your child for school, you will need to provide a certified copy of his or her birth certificate. If your child was born in Texas, you may obtain a certified copy of the birth certificate from the Texas Health Department at 1100 W. 49th Street (458-7111). The cost is $22, and checks are made out to “Vital Statistics”. You can either go to the office for the copy or send for it in the mail. If you mail for it, you should allow 6-8 weeks.
If your child was born in Austin, you can obtain a copy of his or her birth certificate from the City of Austin Office of Vital Records. They can also provide copies of birth certificates for other cities in Texas but ask that you contact them first to be sure that they have access to your child’s birth records. This office is located at 7201 Levander Loop, Building C, Austin, TX 78702-4101 (972-4784) - mailing address PO Box 1088, Austin, TX 78767-1088. Their office hours are 8-4:15. The cost is $23, and checks are made out to “City of Austin”. Please allow 3 weeks if you request the certificate by mail.
You will need to provide the following information when requesting the certificate:
The name on the birth certificate
The date of birth
The name of the city and county where your child was born
The father’s full name
The mother’s full maiden name
Your relationship to the person whose certificate you are requesting
An explanation of why you need the certificate
Your name and address
A COPY of your photo ID (Do not send them your original ID or driver’s license).
Children registering for kindergarten must provide the birth certificate before they can be enrolled. Older children transferring from another school district have thirty days in which to provide the certificate.
